Hands down Jira.

They offer "free" for qualified open source projects at bottom of page.


Check out http://jira.codehaus.org/secure/Dashboard.jspa to see it in
action over at CodeHaus, an open source project which uses it.

I rolled it out at work enterprise wide (+300 developers) and have been
using it for years.  Works just fine with Postgres.  Personally just
love it.
